The Commonwealth Fund Shines Spotlight on PACE Model of Care

As part of an ongoing series on programs that aim to improve outcomes and reduce care costs for patients with complex needs, The Commonwealth Fund examined the model of care of the Programs of All-Inclusive Care for the Elderly (PACE®).

The case study focuses on On Lok, in San Francisco, as the original program and explores the potential for the model to be adapted and spread.

"The PACE experience may offer lessons for policy-makers and health system leaders interested in promoting integrated care for high-need populations," reports The Commonwealth Fund, a private foundation that aims to promote a high-performing health care system that achieves better access, improved quality and greater efficiency, particularly for society's most vulnerable populations.
