About this Role

This is an opportunity for a mission-driven team player who has experience managing home care services to develop and implement home care services for PACE program participants, including the coordination of all Durable Medical Equipment and community-based care. You are a collaborative team member who works as part of the Interdisciplinary Team to create a care plan to help participants’ meet their goals. This is primarily a center-based role.

Expectations for this Role:

• Coordinates Home Care Services for participants using home care agencies
• Participates as a member of the Interdisciplinary Team (IDT), completing assessments, care plans and retrieving information on Home Care Services
• Provides general nursing care in the clinic and eventually, the participant’s home
• Documents in the EMR within set documentation timeline
• Communicates with home care employees to understanding participant’s condition and reports condition, including reactions to drugs and treatments, to the RN and Physician
• Collaborates with the Transportation Coordinator to ensure accuracy for participant transportation
• Schedules Home Care appointments and supports the scheduler for other appointment needs, if needed
• Oversees visits completed by external agencies
• Provides on-call coverage as needed

Attributes of an Ideal Candidate:

• Passion and mission orientation for serving high-risk seniors and frail older adults 
• Experience with frail and elderly adults
• Home Care Experience LVN or RN license preferred
• CPR and First Aid certification (within 60 days of employment)
• Proof of a valid CA driver’s license, personal transportation, good driving record and auto insurance as required by State law
• Must be able to lift and carry or otherwise move up to 25 pounds
