November 28, 2018

The Centers for Medicare & Medicaid Services (CMS) will feature the Cherokee Elder Care Program of All-Inclusive Care for the Elderly (PACE®) in its next long-term services and supports (LTSS) webinar on Wednesday, Dec. 12, at 2-3 p.m. ET.

Attendees will learn how Cherokee Elder Care provides services through PACE, a federal program designed to keep elders living in their homes and connected with their communities. The Cherokee Elder Care PACE Center offers an adult day center, a primary medical care office and a rehabilitation facility.

Representatives of Cherokee Elder Care PACE who will present the webinar are Debra Proctor, program director; Laura Vance, RN, Quality Assurance and Performance Improvement; Thelma Pittman, CFO and marketing manager; Daniel Stout, center manager; John Galdamez, DO, medical director; and Lori Medina, MSW.

The webinar will explain the PACE model of care, identify how the model enhances elders' quality of life with an interdisciplinary team, and compare cost savings between the PACE model and facility long-term care.

You can register online for the webinar. If you have questions for the presenters, email CMS.
