Program Descriptions

Providence ElderPlace – West Seattle

The Providence ElderPlace mission in the words of our caregivers: “Our objective is not simply to grow. It is to transform the lives of those we serve. It is to bring them comprehensive and coordinated care, to bring them relationships, to help them age in place and, when needed, to provide support through end of life.” 

In 1993 the Sisters of Providence in Seattle engaged On Lok, in San Francisco, CA, to assess the feasibility of developing a PACE program in the Pacific Northwest. In December of that year, the Sisters of Providence Board of Directors launched Providence ElderPlace – Seattle, the last of the 15 congressionally approved demonstration PACE projects. Planning for the new program began in 1994, and Providence ElderPlace – Seattle opened its doors on Oct. 1, 1995. Fifteen years later, in a concerted effort to bring PACE to more elders in the community, ElderPlace opened its second center on the campus of a partner long-term care community, Providence Mount St. Vincent, in West Seattle. ElderPlace – West Seattle participants enjoy the shared offerings of the campus, including visits from the children who attend the Intergenerational Learning Center, trips to the coffee shop, and community barbeques. Residents of the on-site assisted living community can transition to Medicaid funding and stay in their apartments by enrolling in PACE. As of July 2022, Providence ElderPlace – Washington operates seven centers in three counties and serves 1,081 participants. Providence ElderPlace – West Seattle offers breathtaking views of the Seattle skyline. We look forward to sharing our unique center with you!

In November of that year, Dr. Assad Kazemi joined ElderPlace as a third-year geriatric fellow, the first to serve and train at ElderPlace. He now serves as medical director for Providence ElderPlace – Washington. As of July 2022, Providence ElderPlace – Washington operates seven centers in three counties and serves 1,081 participants. Providence ElderPlace – Seattle is proud to be located in one of the most ethnically and economically diverse neighborhoods in the Northwest. We look forward to welcoming you to our flagship site!

ICHS PACE – Legacy House 

International Community Health Services (ICHS) offers affordable, skillful and culturally sensitive health services in nearly 70 different languages at 11 locations. Legacy House provides the base of operations for the ICHS Healthy Aging and Wellness Program and the ICHS PACE center. At ICHS Legacy House, elders join a supportive, family-like community that honors and reflects their cultural values. We promote healthy aging under the care of dedicated and knowledgeable staff who speak 15 Asian languages and dialects. Our bright, cheerful facility is located in the heart of the Chinatown-International District within walking distance of public services and shops. 

Assisted Living: Our 75 apartments provide a warm place to call home, with 24-hour care that includes assistance with personal needs and daily activities, such as cooking and managing medications. Residents enjoy nutritious Asian meals, on-site medical clinic and nursing services, and access to recreational activities through our Adult Day Services. 

Adult Day Services: The weekday health and social care program at ICHS Legacy House provides occupational therapy, medication management, meals and more. Recreational group activities such as tai chi, gardening and field trips provide a welcome time to visit with friends. 

Meal Program: A tasty and nutritious Asian lunch is served in a comfortable group setting at the Bush Asia Center. Daily activities include karaoke, bingo, mahjong and origami.
