Job Summary
As a PACE (Program of All-Inclusive Care for the Elderly) Center Nurse Practitioner, you will play a vital role in providing comprehensive and compassionate healthcare services to elderly individuals in our community. Working within a multidisciplinary team, you will assess, diagnose, and manage the healthcare needs of the AgeWell PACE participants, with a focus on improving their overall quality of life. Your expertise and dedication will help seniors maintain their independence and receive the care they need to age in place.
Position Responsibilities
Patient Assessment and Care Management:
- Conduct thorough initial and ongoing assessments of participants, including medical history, physical examinations, and psychosocial evaluations.
- Develop and implement individualized care plans based on assessment findings and participant goals.
- Monitor and manage chronic medical conditions, prescribe medications, and order diagnostic tests as needed.
- Collaborate with other healthcare professionals, including supervising MD, physicians, nurses, social workers, and therapists, to coordinate care and ensure holistic support.
- Participate as an active member of the PACE interdisciplinary team and care planning.
Health Education and Counseling:
- Provide education to participants and their families about healthcare conditions, treatment options, and preventative measures.
- Offer counseling and emotional support to participants, addressing their psychological and emotional needs.
Medication Management:
- Prescribe and manage medications, ensuring participants' medication regimens are safe and effective.
- Educate participants on proper medication administration and potential side effects.
Infection Control and Prevention:
- Implement infection control measures to protect the health of participants, especially those with compromised immune systems.
- Promote vaccination and preventive health measures within the PACE center.
Health Promotion and Wellness:
- Encourage and support participants in adopting and maintaining healthy lifestyles, including diet and exercise.
- Conduct health and wellness assessments and promote preventive care.
Documentation and Reporting:
- Maintain accurate and up-to-date electronic medical records (EMR) for each participant.
- Generate reports on participants' health status, care plans, and progress.
Compliance and Quality Assurance:
- Ensure compliance with all regulatory and accreditation standards related to PACE services.
- Participate in quality improvement initiatives to enhance the overall quality of care provided.
Emergency Response:
- Be prepared to respond to medical emergencies within the PACE center, providing immediate care and coordinating with emergency services as necessary.
Experience & Education
- Master's or Doctorate degree in Nursing with Nurse Practitioner certification.
- Current state licensure as a Nurse Practitioner.
- Geriatric Nurse Practitioner certification or experience working with the elderly population preferred.
- Excellent clinical and diagnostic skills.
- Exercise sound judgement and exhibit strong decision-making and problem-solving skills.
- Detail oriented, with strong multi-tasking skills.
- Excellent writing, proofreading, and editing skills.
- Proficient with software including Microsoft Word, Excel, and Outlook.
- Experience working in a nonprofit, strongly preferred.
- Experience working in a healthcare setting preferred.
- Bilingual in English & Spanish, preferred.

- Physical demands: Employee must be able to sit for extended periods of time. Employee will also be actively present throughout the PACE center, engaged with employees, participants and with staff. Employee is required to use hands and fingers, especially for typing on the computer and using the mouse. Employee must be able to talk and hear, particularly for daily interactions with staff and elderly participants.
- Work Environment: Employee must be able to work in a busy and demanding clinic environment with moderate to loud noise levels.
