Each year, a highlight of the NPA Annual Conference is the opportunity to explore the heart and soul of local PACE programs through exclusive site tours. With this year’s conference being held in San Diego, CA, attendees will be able to visit Family Health Centers San Diego, Gary and Mary West PACE, San Ysidro PACE and St. Paul’s PACE. The PACE programs will host site tours to showcase their facilities, share best practices, and discuss the participant populations they serve. The site tours will be offered throughout the conference, Sunday through Wednesday, Oct. 13-16. Conference attendees can sign up for the tours at the volunteer desk after checking in at registration. Family Health Centers of San Diego (FHCSD) launched FHCSD PACE in the College Grove Shopping Center in July 2019. This innovative center offers a range of services to support seniors, including in-house transportation with door-through-door service, a day center with a licensed recreation therapist, and on-site meal preparation. In October 2022 FHCSD PACE expanded with a second location in Oceanside. The program serves 69 ZIP codes in San Diego County, leveraging the broad network of specialty services provided by Family Health Centers of San Diego. Gary and Mary West PACE, which was established in 2019, is a not-for-profit program funded through a grant from the Gary and Mary West Foundation. The dynamic, research-driven program is committed to transforming senior care in San Diego. West PACE features a 20,000-square foot day center with an expansive, participant-focused space where seniors can engage in tailored activities that promote physical and mental well-being and enjoy nutritious meals. A leader in the community, Gary and Mary West PACE partner with local organizations to reach and support vulnerable senior populations who can benefit from PACE. San Ysidro PACE opened in April 2015 as the flagship site for San Diego PACE. After originally partnering with a primary care SYHealth Senior Clinic, the PACE program expanded to a 12,000-square foot facility, and the clinic was relocated in 2023. San Ysidro PACE has 872 participants, and a second San Diego PACE site, Chula Vista PACE, opened in 2020 and has 916 participants. San Diego PACE currently has four locations, with two additional sites in progress. St. Paul’s Senior Services launched the first PACE program in the region when it opened St. Paul’s PACE in downtown San Diego in 2008. Three more centers opened in Chula Vista in 2012, El Cajon in 2018, and North County in 2020. Each center provides a full range of services to meet the medical, social and emotional needs of low-income seniors with chronic conditions.
