Harbor Health PACE Programs Recognized As Age-Friendly Health Systems

Two Harbor Health Elder Service Plan programs, in Brockton and Mattapan, MA, have been recognized as Age-Friendly Health Systems, an initiative of The John A. Hartford Foundation and the Institute for Healthcare Improvement (IHI), in partnership with the American Hospital Association (AHA) and the Catholic Health Association of the United States (CHA).
The PACE programs were recognized as leaders in a rapidly growing movement “committed to the care of older adults that is guided by an essential set of age-friendly, evidence-based practices across the 4Ms (What Matters, Medication, Mentation and Mobility); causes no harm; and is consistent with What Matters to the older adult and their family.
In letters of recognition, The John A. Hartford Foundation, IHI, AHA and CHA applauded the commitment of the programs. “This is an exciting movement, and there is much to be proud of as together we improve the health and health care of older adults,” the founding partners stated.
