myPlace Health Celebrates Third Anniversary

myPlace Health recently celebrated its third anniversary. The program launched in October 2021 through a joint venture with its founding sponsors, SCAN Group and Commonwealth Care Alliance. Its first PACE center opened earlier this year in Los Angeles, CA, and a second center serving South Los Angeles is expected to open within the next year.

“Over the past three years, we have purpose-built myPlace Health from the ground up in our quest to transform the way older adults age in place in the home they love and in the community they cherish,” said Robbie Pottharst, founding CEO of myPlace Health. “With the support of our sponsors and several others who see PACE as a natural extension of their mission, we believe we are in the early innings of PACE growth at a national level. Our work has just begun.”

myPlace Health serves older adults with complex care needs through the PACE model, which includes a multidisciplinary care team driven by a shared mission to improve the aging experience for older adults. Team members design care plans and organize service delivery around each participant’s life goals and preferences.

myPlace Health integrated behavioral health, palliative care and social engagement in the PACE care model and continue to plan more innovations.
