Advanced Practice Provider: Geriatric

At Alexian PACE, we provide a Program of All-Inclusive Care for the Elderly (PACE). We are seeking to add a BE/BC Primary Care Physician to join our PACE family at our Chattanooga, TN location.

• Obtain patient history and performs assessment via observation, interview and examination.
• Order, perform and interpret diagnostic studies.
• Perform preventative health assessments, screening, immunizations and patient care.
• Provide direct treatment and management of health conditions via referral to other healthcare providers or community resources.
• Counsel and educate patients and their families/caregivers concerning preventative health, treatment options and community resources.
• Document delivery of health care and nursing processes in accordance with specific specialty area standards and network nursing policy. 

Job Duties:

APP works mostly independently but under the direction of a supervising physician.

• Will provide services at Ascension Living Alexian clinic, and could include rounding on inpatients from time to time but not very often.
• Typically see 4-6 patients in a clinic setting  (2-4 routine assessments, plus 2 urgent visits daily)
• Geriatric experience is a plus, requires candidates with at least 3 years of primary care clinic experience.
• Seeking candidates with a strong initiative, great critical thinking skills, emotional maturity and ability to work will within a team setting.
• Willingness to learn new medical knowledge and skills when needed.
• Experience in a geriatric setting, palliative care or hospice setting would be a great asset to our team.
