JOB SUMMARY

The Medical Director is responsible for the implementation of all clinical care across the health care delivery community, clinical outcomes (including family and patient satisfaction), and the implementation and oversight of the quality assessment and performance improvement (QAPI) program. Along with the Clinical Director, The Medical Director leads the design of the Edenbridge PACE program’s model of care, assessment tools, risk stratification processes, and the effective implementation of the interdisciplinary team and care planning process.  The Medical Director serves as the program’s liaison to the local, regional, and national medical and PACE communities. 

As a physician member of the interdisciplinary team, the Medical Director provides direct medical care to program participants during launch and for the program’s early phase, in addition to administrative duties as outlined below. As the program matures and enrollment expands, the Medical Director will also serve as the consultant and resource to a primary care physician (PCP), on-call physicians and nurse practitioners.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Administer the Medical Director’s role in a manner consistent with the program’s core values and mission.
  • Supervise all medical care and services delivered through the program. 
  • Ensure compliance with PACE provider regulations as they pertain to medical/clinical components of care.
  • Evaluate and implement clinical practice guidelines based on the best available clinical evidence, while basing decisions on each participant’s goals and preferences for care, in collaboration with the interdisciplinary team (IDT). 
  • Provide internal consultation to the primary care physician and nurse practitioners, through case review and analysis of program data.
  • Build and maintain strong relationships with physician groups, specialists, contracted providers, hospitals, and skilled nursing/post-acute rehab facilities to strengthen these partnerships and enhance referrals. As needed, proactively address any issues related to the quality of contracted medical services. 
  • Serve as a physician liaison to local, state and federal agencies, as requested.
  • Oversee the Quality Assessment and Program Improvement (QAPI) program through participation on the Quality of Care Committee; design and conduct quality improvement projects and keep abreast of federal Outcomes-Based Quality Improvement initiatives. Develop yearly QAPI Plan, quarterly QAPI reports and year-end evaluation. Ensure the integrity of program data, evaluate program performance relative to national data, and implement adjustments to care plans, processes, and policies as needed to address any deficiencies.  
  • Participate in national quality improvement efforts such as those organized by the National PACE Association, AMDA, and other professional groups. 
  • Monitor medical outcomes and appropriateness of utilization of medical care and hospitalizations. Provide input into cost-effective clinical practices. Stay abreast of research and best practices and work with clinical staff (PCP, NP, RN, etc.) and the rest of the multidisciplinary team as needed to adjust policies and procedures to avoid unnecessary hospitalizations and improve the health of participants.
  • Develop and oversee peer review systems for clinical staff, and coordinate the performance appraisal of the primary care physician. In partnership with the Executive Director, develop and maintain clinic staff job descriptions. Provides staff in-service training as needed. Makes recommendations for hiring/disciplining PACE clinical staff.
  • The director shall complete 24 hours of continuing education training annually to maintain and develop skills. At least two of the required hours of training shall focus on infection control and prevention. When adults with mental impairments participate at the center, at least four of the required hours shall focus on topics related to participants' mental impairments. 
  • Ensure all regulations for Licensed Adult Day Care Centers are met.
  • Additional responsibilities will be added at managers discretion, as career growth opportunities and responsibilities are constantly arising in our flexible and start-up environment.
  • Participates in the role of primary provider at PACE as needed regarding regulatory assessments, care planning, acute episodic care, and managing participant care at external institutions as required.
  • Participates in the off hours and weekend/holiday on-call rotation.

KEY WORKING RELATIONSHIPS

The primary contacts for the Medical Director are the Executive Director, Primary Care Physician, Nurse Practitioner and other members of the Leadership Team.

External contacts include extensive interaction with representatives of the medical community, long term care providers and assisted living providers. In addition, the Medical Director may interact with the community, state, and federal social service, regulatory and other agencies, especially during on-site surveys.

  • Adheres to program policies and procedures, and meets all federal and state licensing laws, certifications, and required regulations.

QUALIFICATIONS

Education

  • MD or DO degree, with current licensure in the Washington DC
  • Board certified in internal medicine or family practice.
  • Completion of fellowship or advanced training in geriatrics (preferred)

Experience

  • Minimum of five years in medical practice for the chronic care population desired
  • Experience working with an interdisciplinary team and performing comprehensive geriatric assessments.
  • A record of forming and implementing a vision for an organization.
  • Demonstrated skills in medical administration and/or previous experience as a Medical Director

Knowledge, Skills and Abilities

  • Enthusiasm for geriatric medicine and community-based care for frail elderly people. 
  • Current knowledge of chronic care/geriatric issues and best practices.
  • Familiarity with population health analytics for the frail elderly population
  • Skill to build strong working relationships with physicians and other health care providers.
  • Good judgment, problem-solving and cognitive skills.
  • Ability to process information accurately and in a timely manner.
  • Ability to coordinate and facilitate teamwork and interdisciplinary groups.
  • Good public communication skills.
  • Demonstrated leadership abilities.
