We are a Program of All-Inclusive Care for the Elderly, which integrates population health management and comprehensive health and social services (operating as LIFE – Living Independently for the Elderly). Community LIFE is committed to helping seniors stay at home safely for as long they are able. Our program brings the region’s experts in geriatric medicine and care together to work as a team in specialized Day Centers, to help older adults enjoy the highest quality of life possible. Our wide range of services are designed to meet the varied needs of seniors, and include medical care, social services, meals, activities, transportation and much more.
Shift: Mon-Fri, 7:30am-4:00pm
Location: Homestead, PA 15120
Summary of Job Duties: Our Nurse Practitioner (NP) is responsible for periodic and episodic assessment and management of individual participants’ medical conditions, and for oversight of their use of medical specialists and inpatient care. The NP is part of an Interdisciplinary Team with the knowledge and skills necessary to assess, plan care for, and provide service to frail elderly participants.
Required Education: Completion of an accredited, Board-approved master’s, postmaster’s or doctoral nurse practitioner program, current, unrestricted license as a Registered Nurse in this Pennsylvania, and current National certification as an Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP) or Family Nurse Practitioner (FNP).
Required Licensure: Pennsylvania RN license, Master level, post Master’s program, or Doctoral degree from accredited Nurse Practitioner program. Active CRNP Certificate in Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP) or Family Nurse Practitioner (FNP), active Pennsylvania Prescriptive Authority Approval and a DEA license. Valid PA Driver’s license.
Required Experience: A minimum of 1 year providing services to a frail or elderly and/or mentally and physically handicapped population preferably in a health care setting or an equivalent combination of education and experience
Required Skills:
- Geriatric Assessment and management of co-morbidity.
- Effective listening and communication skills with patients, families and IDT members.
- Clear and complete documentation.
- Effective Team skills including but not limited to reliability, active listening, positive approaches, courtesy and respect in line with Community LIFE Mission and Values statements.
