Position Summary:

Under the supervision of the VP of Senior Health Services, the PACE Program Director oversees the administration of PACE services for San Diego PACE participants. The PACE Program Director promotes quality, efficiency, and accessibility of all PACE services to PACE participants and families. The Program Director will set expectations for the planning, organization and implementation of PACE Services for San Diego PACE participants, and work in conjunction with the PACE Medical Director to provide strategic and operational guidance for all levels of San Diego PACE staff and contractors, including site managers and directors, staff members, providers, community agencies, health network management, and others who provide care and services to San Diego PACE participants.

Essential Functions of the Job:

  • Oversees the administration of PACE services across the program, including working with other departments and outside agencies to coordinate and integrate services for effective and efficient participant care.
  • Effectively supervises and manages direct reports (per the San Diego PACE Organizational Chart) and provides leadership, support, continuing education opportunities and direction to employees of the organization. Builds an effective team of leaders by providing guidance, career coaching, growth, and personal development to direct/indirect reports.
  • Provides leadership in hiring of PACE staff by ensuring operational and staffing needs are met to meet or exceed monthly enrollment goals per center including determining qualifications, competence, and performance expectations; planning, implementing, and monitoring of staffing plans; providing orientation and training and continuing education.
  • Collaborates with program leaders to review the development of PACE program policies and procedures to ensure PACE staff is educated about changes and workflows are modified.
  • Continuously assesses and improves the department’s performance by designing operational processes, monitoring performance through data, analyzing the data, implementing sustainable performance improvement, and participating in interdisciplinary/interdepartmental improvement activities.
  • Ongoingly reviews financial and qualitative reports to increase understanding of the organization’s financial and non-financial status.
  • Monitors and controls the budget including processes to monitor operational and capital costs.
  • Responsible for the development and oversight of a robust provider network; works closely with the Provider Network Team to ensure all contracted provider agreements are in place and serves as an escalation point for issue resolution.
  • Monitors and evaluates the results of programmatic operations against established objectives, quality assurance/service standards and budget/other financial criteria and reports and submits performance plans as indicated to the VP of Senior Health Services.
  • Oversees the audit process and ensures a plan is created and implemented to address any corrective action measures.
  • Helps set the tone of the department to ensure morale, teamwork, and a positive organizational culture is maintained.
  • Regularly schedules management, department, and staff 1:1 meetings to provide leadership, development, and a forum for communication to ensure coordination and collaboration in meeting organization and individual goals.
  • Establishes and sustains an advocacy role at the local, state, and national levels for community health centers and the patients they serve.

Additional Responsibilities:

  • Works with the PACE Program Medical Director to ensure the Interdisciplinary Team (IDT) is appropriately trained in their role and responsibilities within the team. Supervises and assists Program Managers, Site Directors, and IDT Managers to ensure all CMS/DHCS expectations of IDT are met.
  • Assists in evaluating the IDT and morning meeting structure to increase efficiency and qualitative outcomes.
  • In collaboration with other leaders, creates systems and training to ensure staff is documenting in accordance with CMS and DHCS regulations.
  • Creates qualitative metrics to monitor overall participant satisfaction and addresses operational inefficiencies that could result in dissatisfaction and disenrollment.
  • Ensures PACE centers are operating in compliance with all governing regulations and all other applicable regulatory authorities and protocols.
  • Ensures the delivery of all participant services/programs are compliant with State and Federal rules and requirements.
  • Ensures annual employee performance reviews are conducted in a timely and effective manner.
  • Evaluates performance of employees for compliance with established policies and objectives of the organization and contributions in attaining objectives.

Job Requirements:

  • Education Required (Minimum level of education): A master’s degree in a health-related field or business administration.
  • Experience Required: Five (5) years of experience in a management-level administrative capacity in a health-related organization.
  • Must have PACE experience Three (3) years of experience working in senior or elder care setting – PACE, Nursing Home, Hospice, ADHC.
  • Must Possess the Ability To: Work independently and as an effective member of a team. Comfortable working in a dynamic environment with both short-term and long-term deadlines.
  • Comfortable leading and facilitating discussion with clinicians, patients, and caregivers.
  • Establish and maintain effective interpersonal relationships with all levels of staff, other programs, agencies, members, medical providers, and the general public.
  • Communicate effectively, both verbally and in writing, with individuals from varying cultural and clinical backgrounds.
  • Identify pertinent policy and operational issues; develop strategies, solutions, and recommendations.
  • Multi-task in projects and their respective activities, timelines, and issues.
  • Motivate and lead various participants at all levels in the organization.
  • Effectively utilize computer and software tools.
  • Training and Experience: Strong writing, analytical and oral presentation skills.
  • Candidate must be comfortable working in a dynamic environment with both short-term and long-term deadlines.
  • Knowledge of: Culture and needs of the socially and ethnically diverse population that San Diego PACE serves.
  • Community-based organizations, agencies, and community services.
  • Principles and practices of health care, health care systems, and managed care.
  • Medi-Cal/Medicare benefits and regulations.
  • Techniques of program and policy development, administration, and coordination.
  • Health Plan, Hospital and Medical Group operations and strategies.
  • Knowledge and experience working with LTC facilities, SNF, hospice, etc.
  • Working knowledge of health care delivery and financing systems, including Medicaid, Medicare, waiver programs, prospective and capitated systems, public health programs, and HMO/Managed Care.
  • Good working knowledge of healthcare, social services, and aging networks, as well as relevant federal and state systems.
  • Good working knowledge of the principles, procedures, and techniques governing contract negotiations.
