Colorado PACE has an immediate opening for a Clinic Registered Nurse. Sign-on bonus $3,500.

Location: Aurora, CO

Status: Full-time

Schedule: Monday-Friday, 8:00a - 4:30p

Annual Pay Range: 

Supplemental Pay:  Based on position, schedule and/or availability: Shift Diffs/Stipends

MILEAGE AND EXPENSE REIMBURSEMENT: Sixty-two and a half cents per mile – one of the highest in the industry! Relocation Reimbursement.

REWARDING WORK YOU WILL DO:

The PACE Clinic Registered Nurse is responsible for managing the nursing care essential to the assessment, promotion, maintenance and restoration of participant’s health and wellness within their scope of practice. Will work to maximize the participant’s health outcomes and participant experience by identifying participant needs, completing pre-visit planning, supporting the clinic visit and providing closure to the clinic visits, including scheduling follow up appointments, testing, lab work, and participant instructions. Will respond to walk ins and/or triage calls. Responsible for thoroughly and appropriately completing clinic visits designated as nursing visits.  

ESSENTIAL DUTIES:

  • Demonstrates a working knowledge of best practices in nursing.
  • Confers with team members for effective and timely resolution of participant needs.
  • Maintains flexibility in schedule and responds to unexpected emergencies and changes in daily assignment to meet organizational needs.
  • Serves in the on-call nurse rotation.
  • Establishes therapeutic relationships with participants and families from a variety of cultural, ethnic, and socioeconomic backgrounds.
  • Demonstrates interpersonal skills with all levels of medical, nursing, administrative, and other interdisciplinary team members to effectively and efficiently provide level of service required by participants.
  • Facilitates communication with community agencies and providers including pharmacies, hospital departments, and extended care facilities to optimize participant transitions and the provision of required care services.
  • Completes nursing responsibilities including but not limited to medication reconciliation, and timely completion of required electronic medical records.
  • Effectively evaluates and communicates care, needs, and provides teaching and instruction to meet participant’s care goals.
  • Ensures participant privacy and comfort during their care.
  • Accountable for direct and indirect quality care for the PACE population.
  • Assists in the evaluation of medication compliance and the effectiveness of prescribed medications.
  • Attends and participates in participant/family meetings, and IDT meetings as requested.
  • Maintains confidentiality of all participant events, discussions, and records.
  • Demonstrates compliance with organizational mission, vision and values.
  • Properly cleans durable medical equipment as necessary.
  • Provides for continuity of services in home, community, or clinic through effective information management, both written and verbal communication.
  • Maintains clinical competency and performs duties within scope of practice.
  • Accepts census fluctuations and adapts productivity to support organizational goals.
  • Manages time effectively to encompass direct care, communication, documentation and required meetings.
  • Maximizes utilization of resources within the organization to provide most cost-effective care delivery.

WHAT WE ARE GOING TO LOVE ABOUT YOU:

  • Graduate of an accredited school of nursing; BSN preferred.
  • One year of professional experience required, minimum one year experience working with the frail and elderly population, home care or geriatric experience.
  • Valid RN License.  Valid Colorado Driver’s License and proof of Auto Insurance in accordance with state statute. 

PHYSICAL REQUIREMENTS: Ability to lift/carry a minimum of 35 lbs.

Colorado PACE is affiliated with Care Synergy, a nonprofit network of community-based hospice and palliative care organizations serving Colorado’s Front Range and the Region’s Largest Home Health, Hospice, and Palliative Care Network. Care Synergy provides mission support services to better equip Colorado PACE to operate as a distinct and independent organization while working together to share best practices and serve more Coloradans along the Front Range.
