Blue Ridge Independence at Home is a Program of All-Inclusive Care for the Elderly (PACE).

SOCIAL WORKER – POSITION SUMMARY

Under the supervision of the Social Work Manager, the social workers provides direct social work case management services for members enrolled in PACE. The Social Worker functions as a member of the interdisciplinary team and works with members to increase independence and community integration through a model of self determination.

SOCIAL WORKER - SKILLS AND KNOWLEDGE:

  • Excellent interpersonal communication and phone skills; ability to prepare written assessments and correspondence; ability to read and interpret rules and regulations.
  • Must demonstrate ability to work with a diverse team and flourish in a flexible work environment.
  • Ability to provide psychosocial assessment and individual, family and group counseling.
  • Ability and willingness to work in various locations, for planned activities or special events, family meetings and conferences, admission calls, admissions, and tours.
  • Negotiation skills and ability to carry on several levels of reasoning at once during problem solving sessions; ability to think quickly and to write complex written arguments.
  • Ability to work with participants, families, and others of varying levels of responsiveness and healthcare literacy.
  • Familiarity with word processing, data processing, and computer entry skills to match existing clinical management and other software used by the program.
  • Ability to perform several job functions at once.

SOCIAL WORK - ESSENTIAL FUNCTIONS:

  • Conducts initial psychosocial assessments on prospective program members, and consults with family or other support system members as needed.
  • Develops clear and timely written social service care plans, and implements same for assigned caseload.
  • Periodically reviews social service care plans as needed or required, and revises as appropriate.
  • Maintains regular attendance at, and participation in, interdisciplinary team meetings; collaborates on care planning and service allocation decisions. Provides updates on member changes to Region Functional Screener as needed/indicated.
  • Assists families with problem solving around care-giving issues and serves as the facilitator of family and member meetings as needed.
  • Provides crisis intervention, and advocacy as needed for assigned caseload.
  • Provides supportive counseling to members and families as needed.
  • In conjunction with the interdisciplinary team, coordinates discharge planning for members returning home from hospitals, nursing homes, adult family homes, or group homes; coordinates admissions to group homes, adult family homes, and nursing homes.
  • Assists members with housing issues.
  • Maintains current, well-organized case management records in accordance with social work and Electronic Medical Record (EMR) documentation standards.
  • Assists members with money management as needed.
  • Assists financial eligibility staff or county and state agencies in resolving financial eligibility related issues for assigned caseload as needed.
  • Attends conferences and seminars to maintain State Social Worker certification, and to stay informed of developing trends in geriatric, developmentally or physically disabled social work practice.
  • Develops knowledge of community networks and resources for elderly, developmental or physically disabled population of diverse ethnic and cultural backgrounds, and forms collaborative relationships with same. Provides information to members to assist in establishing and maintaining community links.
  • Advocates on behalf of members for benefits and services through government and community agencies.
  • Assists participants in completing advance directives and guardianship paperwork.
  • Facilitates member enrollment and disenrollment from the program.
  • Conducts home visits and home assessments as needed.
  • Participates in cross-site Social Work Department meetings, other site-based meetings and agency-wide committees.
  • Protects privacy and maintains confidentiality of employee, participant and sensitive agency information.
  • Performs related duties as assigned; including providing temporary coverage for other sites as needed and assigned by your supervisor.

SOCIAL WORKER - QUALIFICATIONS:

Must have a valid driver’s license, proof of insurance and have means of transportation.

Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.

Education: Master’s Degree in Social Work from an accredited school required. Must have an active and up-to-date certification in CPR.

Experience: A minimum total of one year of personal, clinical/internship, and or employment experience providing care or services for a frail or elderly population, and or a developmentally or physically disabled population required. One to two years case management experience preferred.

Hours: Typically, standard business hours (Monday – Friday 8:30 am to 4:30 pm), but may require time beyond standard business hours

Technology: Strong computer skills are required and candidate must demonstrate skills during the interview.
